I'll run you through this on a Mediabook Blu-ray with DVD Germany release:

Start with your title:

Then the formats it will be released on:

(Blu-ray/DVD) if 4k (4K UHD/2D Blu-ray/DVD etc)

 

Then the country of release:

[Germany]

Tag:

1: Type in the tag, usually Multi Retailer for releases or put what exclusive retailer it is.
